I actually think that the little girl in the begaining of Jurassic Park 2 was the last I could remember. It wasn't shown, but it was implied enough.

The book is infinitely better than the movie. *SPOILERS* Robert Muldoon (The Game Warden), Donald Gennero (The Lawyer) & Gerry Harding (The Vet) also live. 5 people die in the first movie, same number as in Jaws. I assume that is a tribute of sorts. And I believe he was talking about Jurassic Park 3. Right on about the book's quality though.
