I actually thought this up in 3 minutes in January, when Undertaker's return ads were being played.

Basically, at No Mercy (PPV before Survivor Series), 'Taker is either the WWE Champion or the #1 contender. The top heel at the time (hopefully it's still Kurt, but it might be Brock again, or even that shitbag Nathan Jones) faces 'Taker at No Mercy, and wins/retains the WWE Championship. After the match, the top heel COMPLETELY decimates 'Taker, much like Show did to him this year around the same time. They could show the parallell between the year, and then say 'Taker is retired for good. NO MORE BAD ASS 'TAKER.

3 weeks before Survivor Series, the top heel WWE Champion shoots his mouth off about how all of his possible contenders for Survivor Series are either injured ('Taker, any other big face that could be injured), or already have matches. Something along the lines of "So, I got the night off at Survivor Series" needs to be said (not word-for-word, but that should be the gist of it).

Suddenly, the lights go out, and a bell is heard. A faint spotlight focuses on the entrance ramp, as mist pours out. The lights come back on, an a casket is at ringside (when the lights are out, and before the spotlight comes on, have security wheel it down, or pull it out from under the ring). The heel just looks at it, and the show closes...

The next week, the top heel is searching for 'Taker all night. But he is nowhere to be found. To end the night, he comes out, looking pale and covered in a nervous sweat, shaking, and calls 'Taker out. The lights go out again, and the bells play. The same faint spotlight focuses on the entrance curtain, where a man in a trenchcoat, head covered with a hat, gray gloves over his hands (crossed over his pelvis, as if in mourning), and shoulder-length hair is standing in front of the Ministry-Cross used in '99 to "crucify" Boss Man and Steve Austin. The man brings one hand up, palm towards the ceiling, and slowly raises his head. The crucifix-like symbol catches fire (although WWE will probably get heat because some dumbasses will consider this a KKK reference), and the lights come back on as the top heel is SCARED SHITLESS. He's sweating, he's shivering, and he can barely move. The man at the top of the ramp brings a mic to his face and says "Rest...in...peace..."

The following week, the top heel hires dozens of security guards, cuts a promo about how he knows it's the Undertaker and that he'll kick his ass at Survivor Series, title or not, and gets challenged to a non-title match that night by the 2nd-top face (probably Edge). The match is okay, with security at ringside to make sure nothing goes wrong. The top heel is just about to hit his finisher when...the lights go out again. The bells ring, and Druids walk out from the sides of the ramp bearing torches. 4 Druids roll a casket out from behind the entrance curtain to ringside, very slowly, and the top heel is paralyzed with fear in the ring as the Druids surroung it. The lights come back on, and security forces all but one of the Druids away. The 2nd-top face (like I said, probably Edge) turns the top heel champ around, hits his finisher, and gets a win. The 2nd-top face rolls out of the ring and walks back up the ramp. The top heel comes to, grabs his belt, and slowly steps onto the ring apron to get a closer look at the coffin. He hops down from the ring apron, and stands to the side of the coffin, staring at it. He opens it, slowly, and finds nothing in there. He slaps the coffin's side, the door still open, and starts laughing. The Druid left behind, waiting by the side guardrail of the ramp, walks up behind the top heel and de-robes. IT'S THE UNDERTAKER, IN FULL 1994 ATTIRE!! The top heel backs up, his belt over his shoulder, still laughing. The top heel bumps into 'Taker, and does a McMahon-esque GULP! The top heel takes a step forward, quivering with fear, and then turns around slowly. 'Taker outstretches his arms, his eyes rolled into the back of his head, and the top heel drops his belt, still shaking with fear. 'Taker looks at the top heel, grabs him by the throat, and then chokeslams him into the casket. 'Taker closes the casket, takes a microphone from the ring announcer, and then stands on top of the casket, holding the WWE Championship high in the air with one hand, and the mic to his mouth in the other as he looks down at the casket beneath his feet. "Survivor Series...Casket Match...Non-Title...You, {top heel's name}, will REST...IN...PEACE..."

'Taker drops the mic and does the one-knee pose with his eyes rolling back into his head as "Graveyard Symphony" plays.

Of course, 'Taker would win at Survivor Series, and then...well, I'm not sure what to do then...

I copy & pasted that from a notepad file I saved it on. I originally posted it at Wrestling-Titans (link in sig) in a thread from back in January.
