I saw Bret Hart do it once. When he returned to enter the ring in over time at WM12. He threw the belt on the ground, jumped in the ring and did the job for Shawn. Bret wasn't a happy camper that night.

I always liked Bret Hart's around the waist and then over the shoulder. He use to say "I wear my belt on the left (shoulder) so it's over my heart."
