A few off the top of my head:

 

Raven getting powerbombed by the Pitbulls in the double dog collar match. Instead of going through the table, the Pitbulls fuck it up and Raven just smacks the back of his head against the edge (kind of like Spike's bump to the outside a few months back). Had I seen it live, I would have thought he was dead.

 

Sabu going for an Asai moonsault onto Cactus Jack and landing stomach first on the rail without bracing himself at all. On the CJ DVD. Foley says Sabu mumbling "Give me time," was the only time he ever heard Sabu speak.

 

Speaking off the Cactus Jack DVD, watch the end of the CJ/Maxx Payne vs. The Nasty Boys. One of the Nastys throws Jack off of the rampway to the floor, and instead of landing on his feet, or even his back, Foley just hits the ground like a sack of potatoes. Just, THUMP.
