2. Azrael - 08/14/04 [Won the title in a three-way with Mysterio and his tag partner Gabriel]

3. Rey Mysterio - 08/15/04 [Won the belt by pinning Azrael in the final part of an 8-man gauntlet match. Summerslam]

4. Teddy Hart - 09/04/04

Vacant - 09/11/04 [Hart disappeared for the week, the belt was vacated and Hart was suspended]

5. Rey Mysterio - 10/10/04 [beat Super Crazy in the finals of a 19-man tournament. October PPV]

X. The Super Sheik - 10/30/04 [His title win was "erased from the books" on November 6th when he revealed himself as the suspended Teddy Hart. Hart's win as Sheik over Super Crazzy in a '#1 contenders match' was also wiped out from the books, so Mysterio was given the belt back and he faced Super Crazzy that night]

Out of all those, Azrael is one hot-shot. Mysterio/Wright/Richards was probably a bad booking move (considering I had a Mysterio/Hurricane-Wright/Richards match with both belts on the line which ended with Hurricane getting pinned)
