First was WrestleMania 9. A case can partially be made for WrestleMania 8 as well. SummerSlam 1992. Full Gear on the back of a hearse. GREATEST ENTERANCE EVER! Ah, OK. It was hot in Sturgis but not IRAQ hot...and Benoit/Melenko managed to have a 30 minute classic that was ignored by the drunk bikers.

I honestly can't think of one casket match that hasn't had a run-in.
