Gus Ferotte headbutting the stadium wall after running for a touchdown, giving himself a concussion that forced him from the game. [Redskins vs Giants, 1997, either Sunday or Monday night. Game ended in a tie.]

This next one is an obscure one, but it's the first and biggest blooper I ever saw live. Back in 1988, Mets vs. Giants at the 'stick. Mackey Sasser is playing catcher for the Mets. He attempts to throw out a baserunning trying to steal. He does everything right formwise. Gets the ball right, pounces up right, makes the proper throwing motion. Only thing is, somehow the ball manages to go virtually nowhere; it falls at his feet.

 

Garo Yepremmian's doomed pass attempt in Super Bowl VII that led to a Redskins touchdown, and proved that even the undefeated 1972 Dolphins weren't "perfect."

I think the same thing happened at a LA Dodgers game as I seem to remember a highlight of Nomo? trying a jumping high kick and the guy who charged the mound did a MMA shoot takedown.
