The true culprit was Immortus. He was somehow manipulating Mephisto (?). He made Mephisto hide two pieces of Master Pandemonium's soul in Wanda's children. Naturally, Pandemonium went after the children and absorbed them into his being. Coincidentally, John Bryne was co-writer of this story.

For some reason, I always preferred the 1996 S.S., mostly because it featured the return of Bret. He had an awesome match w/Stone Cold that helped put him on the map. I know somw are missing, but they wouldn't let me put all of them.
